Anyone who served on the Destroyers HMS Cossack (L03 or D57), plus their spouses and descendants are eligible for membership of the association. To apply for membership, please download the application form (below), print out, complete, and then send to the address provided.
The Application Form is available in four different formats, to try and minimise the difficulties of compatibility. The file sizes do vary: the Word 6 version is the smallest (just 58 Kb),the Adobe PDF file next (62.8kb)and both the Word 97 and Rich Text Format versions weigh in at 132Kb. Just click on the appropriate Link to download:
